Use the -S switch to apxs:
-S CFLAGS=-DSTDAPI
You probably will also need to specify:
-S CFLAGS=-DMOD_PERL -S CFLAGS=-DUSE_EXPAT -S CFLAGS=-O
(which apxs otherwise would set by default).
