If that is the case I recommend that you upgrade to at least 4.0.6
Are you getting an http 404 error? If this is the case try shutting down tomcat and the go into your $Catalina_Home/work/Standalone/localhost and delete the folder that is named ROOT or it could just be an _.
This is where the compiled application is located.

I am restarting tomcat every time I make a change. Also, I try to use index.html instead of index.jsp.
Still no luck. Any other recommendations?
