(For those who didn't read the isapi.log file, the request was
actually for index.html)
What does Tomcat say for this request? Check either the console or
the jvm.stderr/stdout to see what it sees the request as and maybe for some
information about what is going wrong - perhaps you have accidentially
removed the examples context?

I installed TomCat 3.2 on my NT 4.0 platform and got error 404 when tested
it with http://localhost/examples/jsp/index. It seems after ajp12 sent
request, the response returns Not Found message. I tried other urls and got
similar results.
Please take look to the attached file isapi.log.
