The problem is that even though you say "256MB" on the Java Heap, the heap consumes a LOT more than that in Real Ram, and could easily be bouncing off of the 512MB RAM limit of the machine. If there is anything else on this machine I'd be extra concerned. GC and Swap DO NOT mix. Try lowering your Xmx size, and try to get a better grasp on the real image size of your Java process. You could also try added RAM to the machine as well.
