Pain In The ... Just means the effort to reward ratio for fixing this item is high. Most OSS developers (not just Tomcat) tend to gravitate towards either low-effort/high-reward fixes, or exciting new stuff. That leaves high-effort/low-reward fixes like this one lingering in the queue. Unless they're high-reward for someone, who then contributes a patch.
