web.xml doesn't flag anything specific to me. see if you can increase the verbosity of apache or tomcat logging, this might give you more clues.
Not familiar with Liferay. Is it possible to run it all on TC and eliminate apache? It might be a connector problem. I seem to remember a thread about unreliability on RedHat in the last few days, although I didn't pay much attention to it.
