I've only ever seen this when there's an error starting Tomcat.
I never reported this as a 'bug', but if there's a problem processing a
web.xml, you lose the ability to stop Tomcat for some reason. I had it
happening here... it would throw exceptions when I tried to stop it like it
wasn't running...
