Two Tomcat instances = two separate installations of Tomcat. Different server.xml, different work directory, different webapp root.
As far as "when do you need it" that's up to you, there is no hard and fast rule. We use multiple Tomcats because it allows us to segregate our clients' apps, and manage each Tomcat instance separately without interfering with the others.
