You have your 5.25" drive on A and your 3.5" drive on B.  Your BIOS is
probably swapping them so that when booting A is B and B is A.  Tomsrtbt
requires that fd0 be your 1.44m drive.  The bios switch or whatever will
not work.  My recomendation is that you open the box and swap the cable
between the two drives and tell the bios about it.  There is no reason I'm
aware that you want it the way you have it, right now your A drive is the
5.25 and your B drive is the 1.44, but your bios is somehow letting you
boot from B, probably pretending to be A, but not pretending well enough.

If you really want to make a tomsrtbt that will work, it would ONLY work
on this machine, and be broken on all other machines.  You would have to
run the unpack.s script, edit the lilo configuration in buildit.s, edit
the 2/etc/rc.M script, run the buildit.s script, etc.  But the best
solution is just to fix the cable so that your 3.5" 1.44M floppy is really
on A instead of B.
