1. Is there a guide to creating an simulation? I have tried to create my own using a state machine. It doesn't work but I can not find a definitive description of what to do. (The examples work fine.) I do not get any output at all so no visible way to to get information.

There are an user guide and some tutorials in the gforge(http://gforge.enseeiht.fr/frs/download.php/1719/UserDocAndEx.zip), but it lacks a document explaining how to create a simulable model.
To simulate your model, you have to place yours state machines in a class. For all class that have a state machine, you have to create an Instance specification of that class.
All transitions must have a trigger.
A checker will be present in the next release to verify if a model is simulable, it will indicate erros of the model.
2. One of the things I do not see how to do (and maybe it is donw automatically) is the create a "umlevent" element that seems to be a part of every simulation that works.

The creation of an umlevent is only necessary for scenario mode(in interractive, it is automatical), but you can create one(to be loaded in a scenario) by a right click in a project ->New->Other... and select "umlevent model", then click on next, type your file name, click on next, in Model Object, choose "Inject Sim Event" and Finish.
