I found "ODS form". And also the fields indicated.
But the conditions and actions do not appear on the chart. Is that so?

to add action and condition to a transition, just select the
transition, then in the 'ODS form' tab, you will get two
entries, one for the action the other for the condition, under
'[transition name]/DESCRIPTION/PROPERTIES'.
Selecting one of this entries you have an embedded xtext editor open
besides the ODS tree where you can set action and guard.
