your understanding is correct with the addition that onionoo does not
always include the relays own fingerprint in effective_family so it leads
to an inconsistency and can not simply fixed by reducing the counter by
one.
I stumbled on it some more times since reporting it and I find it more
important to fix than I expected initially, but this should probably fixed
in onionoo not relay search.
