Description:
  It is the same trouble as with some other dialogs before.

The file name is displayed with question marks. If these question marks are 
overwritten with a new file name the file gets some garbled name.

I believe resolving this trouble will resolve issue 5 as well. 

You can check both issues with the attached repository bundle.
