Extdiff has always done this.  TortoiseHg was diverging from extdiff
in order to provide that behavior.  In 0.9, in order to add support
for 3-way merge diffs, we starting sharing a bit more code with
extdiff so we may have lost that differentiation in the process.  It
needs to be investigated.

However, if you are skipping the visual diff window, you are directly
using extdiff, and there's
not much thg can do to help.
