I raised this issue yesterday: https://bitbucket.org/tortoisehg/thg/issue/1044/update-to-wrong-patch-when-double-clicking To me it looks like a bug, a) because it's counter-intuitive and b) because it is now impossible to achieve the equivalent of "qgoto X" where X is the second patch from the top of the applied queue.
