Speaking of which, I forgot table of contents/outline details.  For
some reason this isn't coming up on search for me, so I found it via
google.com:

Should we edit that location directly to add to the ToC?  I think
we're ready to outline the Documentation chapter.  Or should we start
using FLOSS Manuals directly as an outlining/editing tool?

I.e., has the upstream for the document move to FLOSS Manuals
directly?  It's sort-of split across two SCM hosts (tos.org MediaWiki
& flossmanuals.net Twiki).
