It also happens on app crashes.

Realised there must be something else to do as well  because i cannot trigger 
it every time anymore.
But I managed to trigger it everytime after reboot  on r144 (3 times in a row)

Few step on how to do > restart phone > phone boots up after wipe you should be 
on app scope.
click on my apps to see all the apps> go to the media player app click on it 
(no video should be available on the device) > message appear saying that there 
is no file to play click ok> bug triggered.

  How to trigger 
  go to the app scop > click on my apps to see all the apps
  go to the media player app click on it (no video should be available on  the
