Just guessing - this could be a leftover from when your phone was RW and
you set the phone to not require a password/code and that got saved in
/etc/ instead of in extrausers - and then the settings app couldn't do
anything because the phone was RO.

Bug description:
Since OTA9 (I think), I can't lock my phone anymore.
I have setup a code, a passphrase, reset my phone, but nothing
works... Unity never ask me for a code...
Another thing that my be related is that pushing "physical" button on
MX4 wake up the phone...
