The error was due to the Theme setting in "Appearance" configuration being left unitialized. The Theme selctor drop down menu was blank. Switching it to a different them and then setting it back to the default reinitializes the Appearance theme and all icons and buttons reappear. Please initialize the Appearance theme to the actual default.
