  Volume controls still unpredictable

Bug description:
  Binary package hint: pulseaudio

  After countless releases of Ubuntu since 2007 and still to this day,
  the volume buttons on my laptop do not make any sense, and I have
  almost no finite control over my volume.

  When I press the volume-up button, PCM and LFE peg themselves to the
  top and my master goes up slowly thereafter.  Unfortunately, this
  causes audio to be extremely loud, or extremely quiet.  No middle-
  ground.

  This has been an issue for a very long time.
