Title:
  [USB-Audio - USB Headphone Set] Switching between outputs sets volume
  to 0 in alsa mixer, causing users to think it's broken

Bug description:
  michael@michael-dell-01:~$ lsb_release -rd

  When changing output settings in System -> Sound, the volume in the
  alsa mixer were set 0%. It took a few hours before I realized the
  volume controls were seperate and I would need to raise the volume to
  100% by using alsamixer -c 1.

  It may be better to amend the routine which initiazes the volume to
  set it to 25% if it does not have a last-setting saved.
