apport hook should provide a Not Applicable choice for all the
questions

Bug description:
Binary package hint: samba
While reporting a bug against samba with apport multiple questions are
asked. The last choice is always Cancel. I'd suggest to add a 'Non
Applicable' choice as well in case there isn't any answer relevant to
the question.
