bluetooth headphones sound to the maximum when connected

Bug description:
when my bluetooth headphones are connected and switched to, the sound starts
playing on the loudest regardles the setting on the volume control. Only when I
change the volume control, it becomes the level it should be regarding the
slider.
This happens on 11.10 but also happened on 10.04
