Bluetooth headphone volume control and media buttons have no effect

Bug description:
When I connect a Bluetooth headset, the tray menu volume controls have
no effect on the volume of the playback and the media buttons on the
headphones have no effect. The speaker output works fine. Note that it
began to just after the upgrade to Impish so it probably is a
PulseAudio regression.
