1: thats just the identifier for the heatsink. It was called SK104 in the heatsink library. I wanted to make sure the heatsink showed up in the schematic since it would burn up without it. 2: I never built a board for these, I just build them on terminal strips screwed into plywood. Really hi-tech! Perfboard would be fine.
