Bit of a story but finally got it working.
Tried setting acct_mgr.* last night but no joy.
Did finally notice that my 'realm' didn't match the case of the
argument I had used previously for tracd (hadn't known what it was at
the time, just copied an example). Fixing that finally showed my
users in the user admin page. But still failed to login.
Tried again today, thinking to switch back to tracd --auth method if
it didn't work. This time I got the login page but logging in just
showed me a blank page.
Wondered if maybe my browser was holding onto something even though
I'd been exiting between attempts so I stopped the tracd service and
tried to bring up the site, expecting it to time out. A bit surprised
when it came up but that was the clue I needed.
I knew not to kill tracd with ctrl-c but hadn't thought about how to
restart it when running as a service. I'm guessing now that
restarting it in the Windows Services manager doesn't do it cleanly
because I had a bunch of python processes running. Restarting the
system did wonders.
I suppose I should test that and if so, add a warning to the wiki
page.
