Without looking at the code again, I believe this is probably a bug. email2trac just hands off the notification part to trac, but it passes ticket information to trac notify based upon what it finds in the email(or at least I believe that is what it does) I don't think email2trac knows about the smtp_always_bcc configuration. There was a release this morning of a new stable version 0.70 which fixed some bugs, but I'm not aware of it being able to fix your problem.
