We have paper-based procedured that call for certain processes to be 
documented, the documents used to execute those procedures, and document 
(procedure) updates to be approved with a date and signature.  I'd like to do 
that in a wiki page.  The nice thing about the wiki is that I can see who made 
every change in the revision history.  And I can have a master list of approved 
versions with links to "wiki:somepage?version=12".  But I'd like to make 
approving an update easy for PHBs.  I started with something like:

But I'd like clicking the Update Form button to fill in the current revision 
when the button was clicked so that over time my table would look like:

 ...||Revision||...
 ...||[wiki:somepage?version=1]||...
 ...||[wiki:somepage?version=2]||...
 ...||[wiki:somepage?version=3]||...

But I can't figure out how to do that with existing tools.  Am I expecting too 
much?  Am I missing a helpful plugin?
