This looks like a configuration problem related to recent changes in
HtPasswdStore options [1] before acct_mgr-0.4, and you missed reading
about it in related wiki documentation. Make sure to look at our
configuration cookbook page as well.

Please re-write

 password_file = /usr2/trac/.htpasswd
to
 htpasswd_file = /usr2/trac/.htpasswd

should help, or you need to tell the exact AccountManager revision/version.

Btw, you may want to select a stronger hash type too:
 htpasswd_hash_type = sha512
