I'm aware of two plugins [1][2], that both use different Trac MacroProcessor syntax for defining working HTML forms in any input field (textarea) supporting WikiFormatting. Each has different strengths and weaknesses, you may want to evaluate on your own [3]. Ask away for more information as required.
