Jonathan You'll see that Julie Thompson answered this question quite specifically with reference to the use of the PLB segment. There isn't a question of whether or not to mix remittance with refunds. The 835 has supported the use of the PLB for as long as I can remember. The PLB is a "provider level adjustment" segment and it contains adjustment info for the provider without any reference to a specific claim in that 835. Payers have long been including provider level adjustments in a PLB when creating their 835's.
