Mark, You are absolutely correct in the fact that once the claims crosses over from Medicare to State Farm, the sending agency (Clearing House in this case or Medicare) has to reformat the transaction to identify State Farm as the destination payer in the 2300 Loop. All provider identifiers in the 2310 and 2420 loop are now specific to the destination payer (State Farm).
