The subscriber's payer identification is whatever collection of payer-assigned
numbers that the payer uses to identify a particular subscriber.  Usually, at a
minimum it is the subscribers ID - #583-92-8650 (for example).  However, for a
variety of reasons, the payer may also require a group or plan number in order to
correctly identify subscriber #583-92-8650.
