Our 834 workgroup decided to include all of the available codes for the race/ethnicity data element when we wrote our addenda to the 4010 IG named for HIPAA usage. This was due to many requests from users who wanted access to the entire code list, not the abridged version we authored in the original 4010 IG. The "Z" code was part of that list, people wanted it, so we included it.

Going forward, our next 834 IG (ver 4050 at this point) will use the new race/ethnicity composite data element (which was not available in the 4010 X12 standards). This will offer maximum flexibility when reporting race/ethnicity in the future.

I can probably help with the second half of your question. The Census Bureau counts race and ethnicity as separate characteristics. The simplest way to translate this would be to consider "race" to be biological heritage and "ethnicity" to be cultural heritage. A person of any race could be Hispanic by culture. Therefor a person could be both Black and Hispanic, or both White and Hispanic. The total White would be the sum of the White (non-Hispanic) and the White (Hispanic).

The problem is that if you total up all the races and those who reported as Hispanic, you have double counted the Hispanics. It is therefor necessary to know how many people reported White, B! lack, etc., as well as how many reported White (non-Hispanic) and Black (non-Hispanic) etc. This problem gets worse if you try to compare Census Bureau data to data in a system that lists White, Black, American Indian, and Hispanic as choices under "race".

The Census Bureau approach is, in my opinion, much more logical and seems to be gaining acceptance as quickly as any standard. Cultural competence is not based on making people choose between what they see as components of their heritage.

The first half of your question is more a coding issue and based on the problem brought about by different systems having different lists of "races". It will be interesting to see the responses to that part of the question.
