2) Can you elaborate on what you mean by Transfer of Service Request.?
 
However based on what I interpret from your Question:
 
At service line level CL1 in loop 2000F covers certification for Adminnsion.
Element CL104 (Situational) on page 188 cover few items.
 
CL104
Code specifying the status of a nursing home resident at the time of service 1354 Use only when certification involves a nursing home resident.

CODE DEFINITION

1 Transferred to Intermediate Care Facility - Mentally Retarded (ICF-MR)
2 Newly Admitted
3 Newly Eligible

6 Temporary Absence - Hospital
7 Temporary Absence - Other
