Go to www.hl7.org, select Special Interest Groups, then select Attachments.  I suspect you'll find a lot of answers there.
 
Someday the first NPRM on all of this plus the X12N materials (www.wpc-edi.com/hipaa) will come.  [The HL7 potential first round HIPAA documents are also available on this web site.]
