Kerry,

Typically you will use the NAIC company code for the payer.  For example, for 
Aetna it is 60054.

For BCBS plans you normally use the three digit Plan Code preceded by "00" to 
make it into a 5 digit code.  Like 00331.

If the payer does not have a NAIC co-code, then you can try their Tax ID 
instead.
