OK, I think I've gathered enough information, so I can try to reproduce
(and of course fix). 
I've opened a bug in order to keep track of the progress of this issue:

So far, it looks like an x264 issue, however.

In the meantime (unfortunately the free time is always too few) you can
try without using b-frames (even by explicitely disabling them) and see
if it's get any better.
