You can buy the cord that connects the handset to the base of a telephone
in any telephone store, so to that extent there is a defacto standard,
without that being part of the North American telephone standard
(TIA/EIA/ANSI-473 ??) 

Different manufacturers may use the same microphones and receivers, but
those are not necessarily standards.  There is  a standard on the acoustic
output of the receiver for a given electrical or digital stimulus and there
is a standard for what the electrical or digital output should be for a
given acoustic input.  (There is more to it than that, but I won't go into
it here).

There are many proprietary telephones used behind PBXs that also meet those
standards but, in eihter case, HOW they do it was never considered an
appropriate field of standardization. 

Buy a copy of the TIA Standard and you may discover as I did many years ago
that there is much more to a plain old black telephone, than meets the eye.
