I believe this could be a destructive latch up caused by an incorrect power 
up sequence (because of transients or other effects) on the different 
supply rails feeding the IC.

Does anyone have any experience, or information, of CMOS 1488 Line drivers
suffering from a sensitivity
to static or transients.  Particularly when the failure mode results in an
almost dead short
between the + & - 12 volt supply rails.?

I am sure I read an article describing how this effect occurs a few years
ago and now, probably because
I am old and grey, I cannot remember where!
