Kevin,

I agree with others that the use of 15-pin D connectors 
for digital applications is limited to North American T1, 1.544 Mb 
applications.  In our gear we use pins 1&9 as transmit (out towards 
the network), pins 3&11 as receive (from the network), pins 8&15 
as chassis ground, and pins 12 and 14 as T1 Remote Loop-Back Tip, minus 
and plus, respectively.

I have not found the source document for the 15 pin D pinout, but the RJ45 
and coax pinouts are specified in ISO/IEC 10173, "Information 
technology - Integrated Services Digital Network primarary access 
connector at reference points S and T."
