I too recommend VolumeCare. I believe it does the same as many other programs, but its UI is the best one. AND it works flawlessly, not ONE crash since day one in my Treo 680.
The boost + volume idea is a very good one, as is the possibility to increase/decrease one or the other (or both) using the side buttons only during the call, AND the ability to set a new default volume on the fly during the call!
