I have heard the if you put vonage in front of everything so that it has the public address and then everything else is behind the vonage box, that it can then handle QOS better. But it is supposed to work behind nat as well. Obviously if you are nated behind a firewall that is blocking the required ports then you will be out of luck if you can't poke the proper holes through.
