If the 4.0 images that are written with the 'dd' command aren't bootable, you may have to use a tool like the usb creator from another version or a tool like unetbootin. The latter is available in a ppa, but there may be free software concerns with using it. Nonetheless, I have done, as a convenience, since the usb creator tool is not accessible using the Orca screen reader.
