Same thing here when using gnash (i.e. when html5 version is not available).  
It started happening recently. It usually starts playing if you change the  
resolution. I usually just use youtube-dl. Annoying problem though and I  
don't know what causes it. I have the flash onoff add-on installed. If I want  
to stream, first I try with it off and if the video won't play I enable flash  
and if the video doesn't load, I change the resolution and it usually makes  
it start.
