I use sqlgrey, provided by TSL RPM. I forgot to mention: The latest amavisd-new has a feature called PenPals. If you configure amavisd-new so use sql for all things if can also keep track on who your users send email to. When a message is received amavisd-new checks to see if you have emailed them recently, and if you have you probably want the reply.
