Thanks for the response Scott.

So I've narrowed down the scenario that causes this
bug:

1) Open browser A and navigate to a form.
2) Open browser B using ctrl-N
3) Submit the form in browser A
4) Nothing happens

Any thoughts as I start to debug this?
