That was it.  I think that the tdk1.1b generated file did not contain this
line, whereas the tdk1.1a9 generated file did.  I switched them over and it
worked first time.

Did you happen to change services.TurbineSecurityService.user.manager in 
your TurbineResources.conf file?  It should read:

services.TurbineSecurityService.user.manager=org.apache.turbine.services.sec
urity.DBUserManager

That's my best guess, unless something else is not getting loaded into 
your classpath properly.  However, any user manager classes should be in 
the turbine JAR file.
