Currently there is a bunch of duplicate code in various places that boots a
runtime. I know of:
* TuscanyRuntime
* TuscanyServletListener
* TuscantContextListener
These all bootstrap in very similar but slightly ways. We should externalize
the bootstrap process so that it can be shared by these implementations
