This patch:
- Deletes the sdo and sca readme.txt files & moves the info into the README
files
- Creates new GettingStarted.html files (with css files in doc/css) to give a
simple, readable starting point that links to the INSTALL, README and
samples/INSTALL files
- Moves the WSEntrypoint documentation from readme.txt to doc/WSEntrypoint.txt
and links to it from GettingStarted.html
- Fixes unclear/incorrect info as pointed out on the tuscany-dev mailing list
- Fixes TUSCANY-525
- Adds the doc files in to the src and bin builds - these have been tested &
work fine on windows, but not yet verified on linux
